Products: Traditional Group Long-Term Disability

Our Long-Term Disability products are designed to help employees protect their families should the employee not be able to work, for an extended period because of an injury or sickness, and not receive a pay check.

Our products offer a variety of plan options, and provide a monthly benefit to eligible employees due to a covered injury or sickness. The employer may fully sponsor the plan, pay a portion of the cost, or the employee may pay the entire cost of the plan.

Definition of Disability Limited Regular Occupation with reasonable employment option and any occupation; with choice of partial or residual disability
Workplace Modification Benefit A benefit to the employer to facilitate the insured's return to work, if the insured is: (1) receiving an LTD Benefit; and (2) able to return to work for the employer should the employer make a modification to the insured's worksite. The standard maximum benefit is $2,000; filed maximum goes up to $25,000
Personal Care Assistance Benefit Provides a monthly benefit to the insured after the end of the maximum benefit period, for a maximum of 60 months, if the insured has a loss of two (2) or more activities of daily living (ADLs), is cognitively impaired, or is terminally ill, and cannot perform any occupation. Monthly benefit is last net monthly benefit, not to exceed $3,000 per month (Optional for plans with benefit durations less than five years)
Vocational Rehabilitation Plan Helping the insured make a successful reentry into the work force by including incentives and services designed to assist disabled insureds to participate in rehabilitation programs
Social Security Assistance Advice and support is available to the insured regarding filing a claim for Social Security disability benefits or appealing a denial of the insured's claim for Social Security disability benefits
Survivor Benefit Provides a lump sum payment that will provide benefits to the insured's eligible survivors in the event the insured dies while receiving disability payments
Exclusions and Limitations Included; A pre-existing condition exclusion is also included
Return to Work Incentive Between 12 and 24 months

Optional Benefits – available for an additional cost

401(k) Contribution Allows employees to continue saving money in their 401(k) account - if they are not working due to a disability;
Pension Contribution Provides benefits to the employer to continue making contributions to an employee's pension plan if the employee is not working due to a disability
Escalation (COLA) Benefit Protects the insured's income against inflation. This benefit provides an annual cost of living increases to the insured's monthly payment based on the CPI-W, after benefits have been paid for 12 months
Spousal Disability Provides benefits for a legally married spouse provided the spouse is disabled under the terms of the policy and not able to perform two or more activities of daily living (ADL), without assistance, has a cognitive impairment, or is terminally ill. Benefits are payable up to $2,000 per month
Additional Catastrophic Disability Coverage Provides benefits to an insured who is disabled under the policy and has lost the ability to safely and completely perform two Activities of Daily Living without another person's assistance or verbal cueing; and have a deterioration or loss in intellectual capacity and need another person's assistance or verbal cueing for his or her protection or for the protection of others. Benefits are payable up o $5,000 per month
Education Benefit Provides an insured who is disabled under the policy and who has a child who is an eligible student, a monthly benefit up to $100 per month for up to two years to help with education costs
Extended Care Benefit Pays an additional benefit to an insured that is confined in an extended stay facility. Benefits are limited to the lesser of $500 or the facilities reasonable monthly charges
Dependent Care Benefit Provides an additional benefit to an insured who is disabled under the policy and who has a dependent that requires dependent care. Benefits are payable up to $300 per month. An eligible dependent includes the insured's child, parent, grandparent, grandchild, sister, or brother related by blood, adoption, or marriage
