Confidentiality Protocols for Victims of Domestic Violence and Endangered Individuals

CDS has established protocols to accommodate requests by victims of domestic violence to receive communications of confidential claim-related information by alternative means or at alternative locations if the person clearly advises that disclosure of all or part of the information could endanger the person or a child.

A person making such a request should submit a written request that (1) contains a statement that disclosure of all or part of the claim-related information to which the request pertains could endanger the person or child, and (2) specifies an alternative address, telephone number, or other method of contact.

Upon receipt of such a request, except with the express consent of the person making the request, we will disclose the following information only in accordance with the requestor’s reasonable request for confidentiality: (1) the address, telephone number, or any other personally identifying information of the requestor or child for whose benefit a request was made, (2) the nature of the health care services provided, or (3) the name or address of the provider of the health care services. The person may revoke his or her request for confidentiality or use of an alternative address or means of communication for claim-related information by writing to us at the address above.
